You should arrive to the pool 5 minutes before your lesson time and have your child ready for his/ her scheduled time. Our pool time is very full so our ability to stay on time is contingent upon our students being on time. If you are late, your instructor will do their best to fit you in, but cannot be guaranteed. When your child has completed his/her lesson, please take the necessary steps to dry off, allow for recovery, redress, and safely exit the pool area so that the next student can arrive and get set up for their scheduled time.

We require that each child not potty trained longer than 6 months wear 2 layers of WASHABLE swim diapers. This "double protection" will help to ensure a safe pool environment for everyone. Disposables are not an option. We do offer diapers at the facility for a cost of $12.00-$20.00 each. Should you not comply with the 2 washable diaper rule and we encounter a fecal incident, you will be responsible for the fees of the lessons canceled after yours. The health and safety of all ISR students is of utmost importance. We will not compromise safety. Please make sure you have a spare washable diaper with you at all times.

Please do not allow your child any food or drinks (exception is water) for at least 2 hours prior to lessons. No one works well with a full stomach and your child will be working hard to learn new skills. No APPLES or apple products, pineapple, papaya, passion fruit, peaches, spinach, honey, celery in any form daily prior to lesson time. The foods listed above can cause the buildup of gas in the abdomen leading to distention and discomfort to your child.

We offer an on-site restroom and we provide a garbage pail for light trash but we ask that you bring ALL diapers home with you (bags by each changing station). We do not have a dumpster so management has to bring home all garbage. Please be sure to check lost & found for any missing items. Items left behind for over a week will be donated to a local organization.

Your child having his/her lesson deserves your attention, support, and cheers. If you have additional children in the pool area, it is recommended that they bring an activity to quietly occupy their lime for the 15-20 minutes you are at the pool. Your cooperation is greatly appreciated and will only add to our efforts in being as efficient and safe as possible. If you allow your child or any child in your care to play in the Facility, including the play yard, you or your designated agent must take full responsibility for any and all injuries that may occur due to said activity and hold harmless Live Like Jake Foundation and any and all agents representing the Live Like Jake Foundation.

Unless it is thundering, lightning, or below 60 degrees in the building, the pool is open. At times weather can make the instructors slightly delayed. If the weather becomes too severe or does not improve, your instructor will have to cancel.

ISR requires 3 towels per day for each child, 2 towels to place on the hand in/out area upon which the child will be laid upon completion of his/her lesson and 1 towel to dry the child with. This policy is in place to prevent the transmission of germs from shared areas of the pool. At the conclusion of the lesson, your child will be placed on his/her left side to allow for proper rest. Please do not leave your child unassisted. Please use extreme caution when placing and removing your child and yourself from the hand in/out station. Safety before, during, and after your child’s lesson is a priority and your ongoing compliance is appreciated.

Please only park in front of our building or across the street in the grass. Please do not park next door.

ASSUMPTION OF RISK: The undersigned states that he/she understands that Live Like Jake Foundation, Inc. and its directors, officers, employees, and agents, are not and shall not be responsible for or liable for any illness, death, injury to person or damage to property arising from, related to, or resulting from the programs provided by the Instructor and the undersigned hereby forever releases, discharges and holds harmless Like Like Jake Foundation, Inc. and its directors, officers, employees, and agents, from any and all claims, liabilities, suits, and/or damages of any kind that the undersigned, the swim student and/or his/her heirs, administrators, representatives, executors or assigns have, may have or claim to have resulting in any way from his/her participation in swim lessons at the Facility.

RELEASE OF LIABILITY: In consideration of being allowed to participate in any way in swim lessons at the Live Like Jake Foundation, Inc. pool, and any related events and activities, I , the undersigned, acknowledge, appreciate, and agree that: The risk of injury from the activities involved in this program to the swim student is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist to the swim student; and, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my minor student’s participation; and, I willingly agree to comply with the stated and customary terms and conditions for my minor student’s participation. If, however, I observe any unusual significant hazard during my presence or my minor student’s participation, I will remove myself and my minor student from participation and bring such to the attention of the Instructor immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the Live Like Jake Foundation, Inc., its directors, officers, employees and agents, (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, to myself and/or to my minor swim student, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law. I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights that I have for myself and for my minor child by signing it, and sign it freely and voluntarily without any inducement.

FOR PARENTS/GUARDIANS OF PARTICIPANTS OF MINORITY AGE - This is to certify that I, as parent/guardian with legal responsibility for the minor swim student, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor swim student’s involvement or participation in the swim lesson programs provided at the Live Like Jake pool, even if arising from the negligence of the Releasees, to the fullest extent permitted by law.

INDEPENDENT CONTRACTOR STATUS – I hereby acknowledge, both for myself and my minor child, that the Instructor is not an employee or agent of Live Like Jake Foundation, Inc., but rather is an independent contractor, without any authority whatsoever to bind or obligate Live Like Jake to any agreements.   I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THESE RULES, ASSUMPTION OF RISK AND RELEASE AND I ACCEPT AND AGREE TO FOLLOW THEM. I ALSO ACKNOWLEDGE THAT THIS IS A BINDING CONTRACT THAT WILL BE VALID FOR THE ENTIRE DURATION OF LESSONS FOR AS LONG AS MY CHILD VISITS LIVE LIKE JAKE.
